從填寫申請表格、各項醫療保險條件要求、說明看護工作項目到安排看護到府服務日期和每週時數，有愛心、耐心的施春美女士都會和您及家人一起商量，以確切能讓老人家獲得最好的待遇和看護。希望華裔社區有老人家的家庭不要錯過這個很好的機會，為老人家帶來理想的看護。

Allow us to introduce ourselves. We are HomeTeam Transition Management Group LLC. We provide in-home services to many clients across the St. Louis area.
Home care agencies enable seniors and those with disabilities to live independently in their own homes or stay with a family member by providing access to outside support care and assistance when needed.
HomeTeam Transition Management Group, LLC has been locally owned and operated in Clayton, MO since 1996. We provide services that are specific to the needs of the client using trained care givers under the supervision of a registered nurse. HomeTeam accepts private duty, Veterans and Medicaid clients. As an approved Medicaid and Veteran provider, HomeTeam closely follows all the requirements mandated by the State of Missouri. We follow these strict standards for every employee we send into a client𠏋 home. All of our employees must pass an annual background check and go through training specific to the home care industry.
HomeTeam provides non-medical, in home services including companion care, respite care, hospital and facility sitting services and end of life care. The tasks that our worker can perform depend upon your individual needs and can include housekeeping, laundry, bathing and personal hygiene, as well as cooking, medication reminders, errand and shopping services. Each plan of care is developed based on your needs together with you, your physician, and our care team here at HomeTeam.
We are pleased to announce the latest addition to our family of caregivers: Chun Mei Liu. Chun Mei is an experienced Care Coordinator and CNA Supervisor with extensive experience in the home care industry. She will personally coordinate and develop a plan of care for you or your loved one. Everything from matching clients with the care team, to assisting with filling out the necessary paperwork for Medicaid or Veterans benefits will be addressed.
